Alternatively, an industrial discharge to the municipal wastewater collection system may require a pretreatment permit.  Form 1 collects general information only. You must also complete a more detailed application based on your proposed discharge activity, as follows: If your facility is a new or existing facility whosedischarge is composed entirely of stormwater associated with industrial activity—excluding discharges from construction activity, you must also complete Form 2F.
