Well Hydrofracture Report - Well Permit Applications
(1) The well driller shall file a Hydrofracture of Well Report to the Approving Authority within 45 days after hydrofracturing the well.
(2) Yield test data shall be submitted with the Hydrofracture of Well Report for both new wells and existing wells.

New (GREEN) - Well Permit Applications
A well construction permit is required for any well that is constructed in the state of Maryland and before installing any well that will explore for water, obtain or monitor may be produced. To ensure a clean and adequate supply of underground drinking water, the state carries out programs to prevent contamination of aquifers from improper the well driller from the local health department.
